CV SERVICES GROUP 401(K) RETIREMENT PLAN Contribution & Match Policy
1. Employee Contribution Rates: Employees may contribute up to 80% of their pre-tax annual compensation.
2. Employer Match/Contribution Conditions: The employer matches 100% of eligible employee contributions up to 5% of compensation. Additionally, employees must have one year of service to be eligible for match contributions. For discretionary contributions, employees must have 12 months of consecutive service, 1,000 hours of service during the Plan year, and be employed on the last day of the calendar year.
3. Vesting Schedule/Eligibility: Participants are immediately 100% vested in their own contributions and the Company's matching contribution. For the Company's discretionary additional contribution, participants are vested after three years of service.
