PARK DENTAL/THE DENTAL SPECIALISTS 401(K) PLAN Contribution & Match Policy
1. Employee Contribution Rates: Employees can contribute up to 80% of their pretax annual compensation. If they do not elect or waive their rights, 6% will be automatically withheld in the first year, increasing by 1% annually to a maximum of 15%.
2. Employer Match/Contribution Conditions: The employer makes a non-elective contribution equal to 3% of eligible compensation to all eligible participants. Additional profit sharing amounts may be contributed at the option of the Companies' Boards of Directors.
3. Vesting Schedule/Eligibility: Participants are immediately vested in their voluntary contributions and actual earnings. Vesting in the Companies' contributions is 50% per year, becoming fully vested after two years of credited service. For other Companies' contributions, participants vest 25% per year starting in the second year, becoming fully vested after five years of credited service.
