SOUTH NASSAU COMMUNITIES HOSPITAL EMPLOYEE DEFINED CONTRIBUTION RETIREMENT PLAN Contribution & Match Policy

1. Employee Contribution Rates: Eligible participants may elect to contribute a portion of their compensation on a pre-tax basis and after-tax (Roth) basis, subject to the Internal Revenue Code limitations for 403(b) plans.

2. Employer Match/Contribution Conditions: The employer contributes a basic contribution equal to a specific percentage of the participant’s compensation, ranging from 3-7% based on eligible Years of Service. Additionally, the employer makes a matching contribution equal to 100% of the participant’s contribution, up to 2% of the participant’s eligible compensation.

3. Vesting Schedule/Eligibility: Participants are vested immediately in their contributions plus actual earnings. Vesting in the Hospital’s contribution portion is based on years of continuous service, with Hospital contributions vesting 100% after three years of service. A year of vesting service is credited for each year in which a participant works at least 950 hours.

4. Roth 401(k) Contribution: Yes, it allows Roth 401(k) contributions.
